Payment & Credit Card Disclosure

Pleasantville Wellness Group accepts multiple forms of payment, including credit cards, debit cards, HSA cards, and electronic payment methods.

Clients are not required to provide a credit card or authorize a card to be kept on file as a condition of receiving services. Payment preferences may be selected at the time services are rendered unless otherwise agreed to in writing.

If you choose to pay for healthcare services using a credit card, please note that credit card payments may be subject to different state and federal consumer protections than other forms of medical payment. We encourage you to ask questions if you would like additional information before selecting a payment method.

Pleasantville Wellness Group does not offer medical credit lines and will not assist with applying for or securing third-party healthcare financing or credit-based payment programs.

Optional Automatic Payment Authorization

As a convenience, Pleasantville Wellness Group offers an optional automatic payment option for balances due after services are provided, including:

  • Session fees

  • Missed or late-cancellation fees (per practice policy)

  • Remaining balances after insurance processing

  • Retroactive insurance adjustments

Automatic payments are voluntary, processed only after services are rendered, and may be revoked in writing at any time, subject to any outstanding balance.
